1. Document proving that 5510 insurance has been taken out (to be obtained from e-government)
2. Transcript or copy of participation certificate showing completion of a 16-hour Occupational Health and Safety Training course within the last year
3. Test results showing immunity to the following tests; if immunity is not present, the necessary vaccinations must be administered and the tests repeated.
